The Ohio Department of Health, School Immunization Requirements include a dose of Tdap (tetanus-diphtheria-pertussis) vaccine and a dose of Meningococcal (MCV4) vaccine to be administered before a student enters the seventh grade. A Tdap is given in order to prevent pertussis (whooping cough). The Meningococcal vaccine is a NEW requirement that became in effect a couple of years ago.
As with other Ohio school immunization requirements, students who DO NOT provide written documentation of these immunizations are subject to exclusion from school after Septermber 4, 2018. The complete date and healthcare provider signature or stamp are required as written proof of vaccination.
