Entering 7th Graders Tdap Booster and Meningococcal Vaccine Requirements
The Ohio Department of Health, School Immunization Requirements include a dose of Tdap (tetanus-diphtheria-pertussis) vaccine and a dose of Meningococcal (MCV4) vaccine to be administered before a student enters the seventh grade. A Tdap is given in order to prevent pertussis (whooping cough). The Meningococcal vaccine is a NEW requirement as of this year.
As with other Ohio school immunization requirements, students who do NOT provide written documentation of these immunizations are subject to exclusion from school in August 2017. Your current sixth grader will need to show proof of having received both of these vaccinations before they can return to school in the fall. The complete date and healthcare provider signature or stamp are required as written proof of vaccination. Attention Parents of 11th grade students:
The Ohio Department of Health released the new immunization schedule for the 2016-2017 school year. All students entering 12th grade in 2017 will be required to have (2) Meningococcal (MCV4) Vaccines prior to entry.
Once your child has received these immunizations or if your child has already received these immunizations, please provide a copy of the vaccine record from the physician’s office to the school nurse. You may have the physician’s office fax the immunization record to Triway High School, fax at 330-262-2620. Or send it via email to firstname.lastname@example.org
